Acclaris Reimbursement Form - Use this form to submit your claims for qualified medical expenses.
Aetna Rx Home Delivery Form - Use this form to enroll in Aetna's Rx Home Delivery service.
Special Claims Form - Use this form for claims incurred when you have emergency or urgent medical care while traveling outside of the United States for up to six months per year. You will need to pay for your health care in these emergency situations, and after you file your claim, Aetna will reimburse you for its share.
